1) I/We, the Parents/Legal Guardians of __________________________ (Camper) do hereby authorize Copper Basin Bible Camp, its employees, staff, volunteers, directors, or others acting on its behalf as agents for the above minor child to consent to any x‐ray examination, anesthetic, medical, or surgical diagnosis or treatment at medical care facilities (i.e., hospital, urgent care, doctor's office) which is deemed advisable by any physician or surgeon for my child as named above. I further accept the financial responsibility for all medical attention, which may be needed so long as a licensed and qualified physician or surgeon prescribes this medical attention (i.e. prescribed medicines, x‐rays, medical imaging, or any other procedures).
2) I/We do hereby agree to release, indemnify, and hold forever harmless Copper Basin Bible Camp, its employees, staff, volunteers, directors, or others acting on their behalf or assigns against loss from any and all claims, demands, or actions in law or in equity that may hereafter be made or brought by the said minor child or by anyone on behalf of said minor child for the purposes of enforcing a claim for damages on account of any fatality and/or injury incurred in consequences of an accident that may be sustained by said minor child at Copper Basin Bible Camp.
3) I/We hereby acknowledge the photographing, videotaping, and other relevant media sources of my child at camp, and consent to the use of those images in the Camp scrapbook, advertisement, website, Facebook, and promotion of Copper Basin Bible Camp.
4) I have read, understand and reviewed ‐ with the Camper ‐ all camp rules and standards of conduct. I understand that violation of these standards may result in my child being sent home at my expense.
Camp Rules & Standards of Conduct
At Copper Basin Bible Camp, every Camper will have the opportunity for a safe and uplifting Christian camp experience. To aid in that opportunity, Copper Basin Bible Camp has enacted these rules to ensure the best Christian camping experience possible:
1. Treat others as you would like to be treated ‐ Matthew 7:12.
2. Look not only to your own interest, but also to the interests of others ‐ Philippians 2:4.
3. Each Camper is expected to participate in all camp activities (unless excused by Camp Director).
4. No leaving the camp property without express permission of the Camp Director ‐ CBBC 1:1.
5. No foul language ‐ Ephesians 5:4.
6. No alcohol, drugs, or tobacco of any kind ‐ 1 Corinthians 6:19‐20.
7. No electronic devices. (This includes cell phones)
8. No knives or weapons of any kind.
9. No matches, lighters, or fireworks.
10. No food in cabins ‐ Critters 8:1.
11. No personal use of the camp phone.
12. All injuries or illness must be reported to the Camp medical advisor.
13. No pets.
14. There must be no personal display of affection between Campers and no splitting off from the main group.
15. Boys are not allowed in girls' cabins; girls are not allowed in boys' cabins.
16. No rough housing, practical jokes, or fighting.
17. Camp property should be treated respectfully.
18. Modest dress is expected at all times. No tube tops, spaghetti straps, halter tops, sleeveless shirts, bare midriffs, mesh shirts, or muscle shirts. All pants must be loose fitting and no shorter than 4" above the knee. No holes in pants above the knees. No clothing with inappropriate language, designs, or insignias.
19. Personal hygiene rules must be observed (i.e., showers & clean clothes daily).
20. Personal effects and luggage may be searched in the presence of the Camp Director or medical advisor for illegal items, offensive items, or in an emergency situation.
21. Campers must stay out of the creeks and ponds in unsupervised situations.
